Cancer Care Ontario - Models of Care Program

Cancer Care Ontario’s Models of Care Program aims to reduce the incidence and prevalence
of cancer, provide timely and appropriate care, provide strategic human resource planning, and
improve patient experience through better integration and coordination of care.

One of the objectives in Phase 2 of the Models of Care Program is to define and derive effective
use of the advanced practice nursing roles with the goal to improve system sustainability and
patient-centredness, while maintaining or increasing quality, patient and provider satisfaction.

Combined with other elements of the model such as working with key stakeholders and
improving processes and tools to identify and meet health human resources needs, the Program’s
ultimate vision is to establish a sustainable workforce providing cancer care in a patient-centred
model.
